Graphnet Health Wins Jersey Contract for Island-Wide Remote Monitoring Service

Graphnet Health has been chosen by Jersey's Family Nursing & Home Care to deliver the island's first large-scale remote monitoring service.

This groundbreaking project, funded through the Impact Jersey CareTech Challenge program and facilitated by Digital Jersey, marks a significant step forward in healthcare innovation.

The contract will enable Family Nursing & Home Care (FNHC) to utilize Graphnet's Remote Monitoring solution, powered by Luscii, to monitor the health of frail and vulnerable islanders in their homes and community settings. This initiative is part of a substantial £800,000 CareTech grant awarded to FNHC for implementing a digital monitoring system for older residents in Jersey.

Rosemarie Finley, Chief Executive of Family Nursing & Home Care, shared her insights: "As a community nursing organization, we witness firsthand the profound impact of frailty, falls, poorly managed wounds, and long-term conditions like diabetes on our patients and their families."

"This project equips our teams with a powerful new tool to closely monitor individuals in their own homes, all without placing unnecessary strain on patients or staff. By leveraging Graphnet's remote monitoring technology, supported by the CareTech Challenge funding, we will be able to detect early warning signs, intervene more promptly, and assist more islanders in maintaining their independence, safety, and well-being for extended periods."
